    Parent Actions to SUPPORT FUNDING
  • Support the monthly 98% attendance goals.  Send your student(s) EVERY day and on time! Make attendance a priority for Head
    Start, tutoring, EDGE clubs and other programs.
  • Support school fund raisers.
  • Early dismissals impact your child’s attendance and the school’s attendance rate.  Early dismissals may result in only a half day
    of attendance. Therefore, your student will be ineligible for monthly and end of the year perfect attendance incentives and awards.
  • Submit your lunch forms by September 2.  We need 100% compliance.
  • Submit school physicals/immunizations ASAP.  If needed, participate in the Sept. 21 school site immunization program at
    Revere.  Just sign a Health 4 Chicago consent form.

    Parent Actions that HONOR SAFETY AND SCHOOL RULES
  • Send your child to school in uniform colors (navy bottoms, white/light blue collared tops).  
    No pink, purple, red, etc. tees.  Hair colors MUST be natural tones.  
  • Purchase the school gym uniform.
  • Remind students not to use their cell phones without permission.
  • Review the Student Code of Conduct (SCC).  Help your child understand the consequences for bullying, cyber bullying, sexual
    harassment, food fights, fighting and disorderly conduct in the classroom, halls, lunchroom, auditorium, etc.
  • Always enter through the main entrance, sign at the desk and get a pass.
  • Please respect instructional time.  Be prompt for conferences.  Conferences must end at 8:50.  
  • Except for conferences, there will be NO admittance into the building from 8:30-8:45A and from 2:30-2:50P.  
  • Parent-teacher conferences must be held between 8:30-8:50 a.m. or by a special arrangement that DOES NOT interfere with
    instruction.

    Parent/Community Action to INCREASE INVOLVEMENT AND STUDENT ACHIEVEMENT
  • Volunteer in the classroom, for the breakfast/lunch programs and/or parent patrol.
  • Attend grade level parent meetings so that you understand promotion policies, grading, and the Student Code of Conduct.
  • Make sure your students (ALL GRADES) read 30 minutes each night.
  • Check your student’s book bag regularly. Please sign and/or send required documents (i.e. permission slips, lunch forms, parent
    contracts, etc.)
  • Attend monthly LSC and NCLB/PAC meetings.  See flyers and bulletin boards.
  • Attend parent report card conferences on Nov. 2, 2011 and Apr. 18, 2012.
  • Provide the school with accurate contact information two (2x)/year. Required by CPS.
  • Attend family literacy/technology and math nights.
  • Ask your child to show his/her progress reports on  Sept. 14, Dec. 7, 2011 and Mar. 7 and May 23, 2012
  • Log on to Grade Book Parent Portal weekly and check your child’s grades. Go to: https://parent.cps.k12.il.us.   Contact the
    school if you need your child’s PIN and CPS ID numbers. You must be the legal guardian to access this information.
